Patients are seen by appointments only, so please call in advance so that we may reserve a time for you. We make every effort to be on time for our patients, and ask that you extend the courtesy to us.
If you cannot keep an appointment please notify this office immediately by calling. Please give us at least 48 hour notice. This courtesy on your part makes it possible to give an appointment to another patient who desires to see the doctor.
We believe in the importance of regular preventative dental health care. However, the responsibility of returning for this treatment lies with you, the patient. Before you leave the office at the conclusion of your current visit we will always try to schedule your next appointment 6 months in advance. This guarantees that you will have an appointment reserved at the proper time.
When you return for your recall visit, please advise the receptionist of any change in address, telephone number, marital status, employment, or insurance coverage.
Every effort is being made to keep down the cost of dental care. You can help us by paying at the time of your visit. Payment can be made by cash, check, Visa™, Mastercard™, or Discover™. If treatment will require several visits, you will be given an estimate and asked to make definite financial arrangements. Remember, even if you have insurance coverage, you are still responsible for the payment of your account.
Please obtain an insurance form from your place of employment and complete your section requiring personal information and signatures. We will then complete and submit your form for you. This is a service provided for you at no additional charge.
Any questions about insurance should be discussed with our office staff. We welcome and encourage discussion of services and fees prior to treatment in order to avoid misunderstandings. Please remember that the relationship is between you, the insured patient, and your insurance company.
Insurance is intended to cover some, but not all of the cost of your dental care. Most plans include either coinsurance, a deductible, "a table of allowances", "schedule of benefits", or "UCR" as well as other expenses which must be paid by the patient. The tables are not fee schedules, although they may sometimes be called this, but rather a list of amounts allowed by the insurance company that go toward the actual fees. The amounts vary depending on the terms of your contract.
Should you wish to determine the benefits to which you are entitled under the provisions of your contract, we recommend that you contact your insurance company, employer, or union to obtain the correct information about your coverage.
In the event you do not receive the benefits to which you believe you are entitled from your insurance carrier, we suggest that you contact them and your employee benefits representative to request a review of your claim. If we can assist you in any way, we will.
